COVID-19 Protocol


Thank you for entrusting Southlake Family Eye Care with your vision and eye care needs! We know you are aware of the constantly changing concerns about COVID-19 and want to take this opportunity to update you of the precautions our office is taking to ensure the safety of our patients and staff.
 
Our goal is to ensure a safe and sanitary environment for everyone in our office. If you have any symptoms of the coronavirus (cough, respiratory distress, fever, headache, loss of taste/smell, shortness of breath, chills, shaking, sore throat, muscle pain), we ask that you reschedule your appointment by calling (817) 481-1665. If you have been in close contact with a COVID-19 patient or have traveled out of the country or to a domestic hot-spot recently, we ask you to reschedule your appointment as well.
 
We are strictly following CDC, the State of Texas, and local guidelines. This includes, but is not limited to:


  • We are seeing patients by appointment only.
  • We are collecting insurance information through your encrypted patient portal prior to your appointment.  We collect both medical and vision insurance to maximize the amount of money those companies pay for your exam and minimize the amount you pay out of pocket.
  • Patients must arrive with face protection/mask.
  • We are limiting the number of people in our office. We ask that only the patient (unless the patient is under 18, has    communication barriers, or is disabled) come inside the building.
  • A front desk hand sanitizing station is available for your use upon entering.
  • All staff are equipped with face masks and/or face shields.
  • Our water fountains and children’s playroom are closed.
  • All surfaces are being cleaned and disinfected routinely.
  • All instrumentation, chairs and exam rooms are disinfected after every patient.
  • Our frame gallery is open; every frame touched/tried on will be sanitized before being placed back on display.
  • Please bring your glasses, readers and sunglasses to your appointment. We will clean and sanitize them for you!
